Financial Forms

For questions about giving, including obtaining a giving statement, contact our financial secretary at glcfinancialsecretary@gmail.com.  For questions about expenses, reimbursements, or church budget, contact our treasurer at glctreasurer@mail.com.

  • Gift in kind form: complete if you’ve given a gift “in kind” a donation of a physical item, and have the receipt or documentation.  This makes it a part of your giving record, and helps us better understand the true cost of our ministries.
  • Reimbursement/Check request form:  For reimbursement or a check request for approved/budgeted ministries.  Requires an invoice, receipt, or documentation.
